ABSTRACT

INTRODUCTION@#Normal stress myocardial perfusion imaging (MPI) carries a favourable prognosis. Conversely, elevated coronary artery calcium (CAC) is associated with increased major adverse cardiovascular events (MACE). There is limited information on the prognosis and management of patients with elevated CAC and normal MPI. We aimed to assess the outcomes of patients with elevated CAC and normal MPI in relation to post-MPI statin use.@*METHODS@#A retrospective review of normal MPI with CAC score >300 was performed between 1 March 2016 and 31 January 2017 in a Singapore tertiary hospital. Patients with known atherosclerotic cardiovascular disease or left ventricular ejection fraction <50% on MPI were excluded. Patient demographics, prescriptions and MACE (cardiac death, nonfatal myocardial infarction and/or ischaemic stroke) at 24 months after MPI were traced using electronic records. Binary logistic regression was used to evaluate for independent predictors of MACE.@*RESULTS@#We included 311 patients (median age 71 years, 56.3% male), of whom 65.0% were on moderate to high-intensity statins (MHIS) after MPI. MACE was significantly lower in the post-MPI MHIS group (3.5% vs. 9.2%, P = 0.035). On univariate binary logistic regression, post-MPI MHIS use was the only significant predictor for MACE (odds ratio [OR] 0.355 [95% confidence interval (CI) 0.131-0.962], P = 0.042), even after multivariate adjustment (adjusted OR 0.363, 95% confidence interval 0.134-0.984, P = 0.046).@*CONCLUSION@#Post-MPI MHIS use is associated with lower MACE and is an independent negative predictor for 24-month MACE among patients with normal MPI and CAC >300.

BACKGROUND: Exercise tests are an important tool in the investigation of myocardial ischemia. The ramp protocol has gained increasing importance in clinical practice because of the possibility of individualizing its exercise intensity. OBJECTIVE: To assess and compare the sensitivity, specificity, and accuracy of Bruce and ramp protocols for exercise testing in the diagnosis of myocardial ischemia considering myocardial perfusion scintigraphy as the reference standard. Secondary objectives included the assessment of hemodynamic profiles, functional capacity, and the incidence of arrhythmias in each of the protocols. METHODS: Participants underwent exercise testing using the ramp and Bruce protocols, and the tests' diagnostic power was assessed. For testing the difference between data provided by both protocols, we used a paired Student's t-test or Wilcoxon test, depending on the assumption of data normality. The level of significance adopted for all tests was 5%. RESULTS: The ramp protocol showed sensitivity, specificity, and accuracy values of 55.6%, 82.4%, and 76.7%, respectively, whereas the Bruce protocol had results of 77.8%, 64.7%, and 67.4%, respectively. The maximum heart rate and double product at peak exercise were significantly higher in the Bruce protocol (p = 0.043 and p = 0.040, respectively). No differences were observed between the incidence of arrhythmias in both protocols. CONCLUSION: The Bruce protocol presented higher sensitivity for detecting ischemia on the exercise test, while the ramp protocol presented higher specificity and accuracy.

ABSTRACT

Objective: To investigate the diagnostic efficiency and incremental value of quantitative myocardial blood flow measurements by Cadmium-Zine-Telluride (CZT) single photon emission computed tomography (SPECT) dynamic myocardial perfusion imaging (MPI) in patients with coronary artery disease (CAD) compared with traditional semi-quantitative measurements by MPI. Methods: This is a retrospective, cross-sectional study. We retrospectively analyzed clinical data of patients with suspected or known CAD, who underwent the dynamic MPI quantitative blood flow measurement of CZT SPECT in TEDA International Cardiovascular Hospital from October 2018 to December 2020. Clinical data, semi-quantitative parameters (stress score (SS), rest score (RS) and different score (DS)) and myocardial quantitative blood flow parameters (rest myocardial blood flow (rMBF), stress myocardial blood flow (sMBF) and myocardial flow reserve (MFR)) were analyzed. According to the results of coronary angiography, patients were divided into the stenosis group and the control group with coronary artery stenosis ≥50% or ≥75% as the diagnosis criteria. The differences of quantitative and semi-quantitative parameters between the two groups were compared, and the diagnostic efficacy was compared by receiver operating characteristic(ROC) curve. Results: A total of 98 patients with a mean age of (62.1±8.7) years were included in the study, including 66 males (67%). At the patient level, with the positive standard of coronary artery stenosis≥50%, the left ventricle (LV) stress MBF (LV-sMBF) ((1.36±0.45) ml·min-1·g-1) and LV-MFR (1.45±0.43) of the stenosis group were lower than the LV-sMBF ((2.09±0.64) ml·min-1·g-1) and LV-MFR (2.17±0.54) of control group; summed SS and summed DS were higher than control group (all P<0.05). With the positive standard of coronary artery stenosis ≥75%, the LV-sMBF ((1.19±0.34) ml·min-1·g-1) and LV-MFR (1.34±0.35) of stenosis group were lower than the LV-sMBF ((1.94±0.63) ml·min-1·g-1) and MFR (2.00±0.58) of control group; all semi-quantitative parameters were higher than control group (all P<0.05). At the vascular level, with coronary artery stenosis ≥50% as the diagnosis criteria, the sMBF ((1.26±0.49) ml·min-1·g-1) and MFR (1.35±0.46) of stenosis group were lower than the sMBF ((1.95±0.70) ml·min-1·g-1) and MFR (2.05±0.65) of control group; SS and DS were higher than control group (all P<0.05). With coronary artery stenosis≥75% as the diagnosis criteria, the sMBF ((1.12±0.41) ml·min-1·g-1) and MFR (1.25±0.38) of stenosis group were lower than the sMBF ((1.84±0.70) ml·min-1·g-1) and MFR (1.93±0.66) of control group; all semi-quantitative parameters were higher than control group (all P<0.05). With coronary artery stenosis≥50% as the diagnosis criteria and CAG as the reference standard, the AUC and 95%CI of myocardial quantitative blood flow parameters indicated by ROC curve for diagnosis of CAD were 0.830 (0.783-0.877). The sensitivity (86.1% vs. 61.5%), specificity (82.6% vs. 73.8%), positive predictive value (77.8% vs. 62.5%), negative predictive value (89.3% vs. 73.0%) and accuracy (84.0% vs. 68.7%) were all higher than the semi-quantitative parameters (all P<0.05). With coronary artery stenosis≥75% as the diagnosis criteria, the AUC and 95%CI of myocardial quantitative blood flow parameters indicated by ROC curve for diagnosis of CAD were 0.832(0.785-0.879). The sensitivity (89.2% vs. 67.6%), negative predictive value (95.5% vs. 86.2%) and accuracy (80.6% vs. 68.0%) were all higher than semi-quantitative parameters (all P<0.05). Conclusion: Compared with traditional SPECT MPI derived semi-quantitative parameters, diagnostic efficacy for CAD is higher using CZT SPECT quantitative myocardial blood flow parameters, this strategy thus has additional diagnostic benefits and incremental value on the diagnosis of CAD.

ABSTRACT

Fundamentos: O papel da cintilografia de perfusão miocárdica em pacientes assintomáticos permanece restrito a situações clínicas muito específicas, muitas delas abordadas nos Critérios de Uso Apropriado (AUC) de Cintilografia de Perfusão Miocárdica. Objetivo: Realizar uma análise crítica da aplicação desses critérios nas indicações de exames realizados em pacientes assintomáticos do Instituto Dante Pazzanese de Cardiologia, cuja população é notadamente de alto risco cardiovascular. Métodos: Foram selecionados pacientes assintomáticos que realizaram cintilografia de perfusão miocárdica para pesquisa de isquemia. As indicações dos exames foram classificadas em apropriadas, inapropriadas ou incertas. Hipocaptação fixa, hipocaptação transitória ou dilatação isquêmica transitória foram consideradas exames alterados. Na análise estatística, buscou-se avaliar a correlação entre o grau de recomendação das indicações e a presença de exames alterados. Resultados: A partir de uma seleção inicial de 2.999 prontuários, 490 foram considerados assintomáticos e incluídos conforme critérios de inclusão estabelecidos previamente. Apenas 9,8% das indicações foram inapropriadas, enquanto que 61,4% foram apropriadas, e 28,8% foram incertas. A hipocaptação fixa do radiofármaco ocorreu em 43,5% dos casos e a hipocaptação transitória, em 16,1%. Solicitar o exame de maneira apropriada ou incerta foi fator preditor de exame com resultado alterado nesta população. Conclusão: O uso dos critérios de uso apropriado da cintilografia de perfusão miocárdica mostrou-se eficaz em predizer exames alterados em uma população assintomática de alto risco cardiovascular, especialmente no grupo de pacientes com indicação incerta, o que pode significar que algumas das indicações consideradas incertas talvez sejam apropriadas para uma população de alto risco cardiovascular. Abstract Background: Functional assessment to rule out myocardial ischemia using coronary computed tomography angiography (CCTA) is extremely important and data on the Brazilian population are still limited. Objective: To assess the diagnostic performance of myocardial perfusion by CCTA in the detection of severe obstructive coronary artery disease (CAD) compared with single-photon emission computerized tomography (SPECT). To analyze the importance of anatomical knowledge to understand the presence of myocardial perfusion defects on SPECT imaging that is not identified on computed tomography (CT) scan. Method: A total of 35 patients were evaluated by a simultaneous pharmacologic stress protocol. Fisher's exact test was used to compare proportions. The patients were grouped according to the presence or absence of significant CAD. The area under the ROC curve was used to identify the diagnostic performance of CCTA and SPECT in perfusion assessment. P < 0.05 values were considered statistically significant. Results: For detection of obstructive CAD, CT myocardial perfusion analysis yielded an area under the ROC curve of 0.84 [a 95% confidence interval (CI95%): 0.67-0.94, p < 0.001]. SPECT myocardial perfusion imaging, on the other hand, showed an AUC of 0.58 (95% CI 0.40 - 0.74, p < 0.001). In this study, false-positive results with SPECT are described. Conclusion: Myocardial perfusion analysis by CTA displays satisfactory results compared to SPECT in the detection of obstructive CAD. CCTA can rule out false-positive results of SPECT.


Resumo Fundamento: A avaliação funcional para descartar a isquemia miocárdica utilizando a angiotomografia computadorizada (angio-TC) de coronárias é de extrema importância e dados na população brasileira ainda são escassos. Objetivo: Avaliar o desempenho diagnóstico da perfusão miocárdica pela angio-TC de coronárias na detecção de doença arterial coronariana (DAC) obstrutiva significativa em comparação com a tomografia computadorizada por emissão de fóton único (SPECT; do inglês, single photon emission computerized tomography). Analisar a importância do conhecimento anatômico para entender a presença de defeito de perfusão miocárdica pela SPECT que não é identificado pela tomografia computadorizada (TC). Método: Trinta e cinco pacientes foram avaliados por um protocolo de estresse farmacológico simultâneo. O teste exato de Fisher foi utilizado para comparação entre as proporções. Os pacientes foram agrupados de acordo com a presença ou não de DAC significativa. A área sob a curva foi utilizada para identificar o desempenho diagnóstico da avaliação da perfusão pela angio-TC de coronárias e pela SPECT. Os valores de p < 0,05 foram considerados estatisticamente significativos. Resultados: Para detecção de DAC obstrutiva a avaliação da perfusão miocárdica pela TC teve uma área sob a curva de 0,84 [intervalo de confiança de 95% (IC95%): 0,67 a 0,94, p < 0,001]. Já o estudo da perfusão miocárdica pela SPECT foi de 0,58 (IC95%: 0,40 a 0,74, p < 0,001). Neste estudo, foram descritos falso-positivos pela SPECT. Conclusão: A avaliação da perfusão miocárdica pela angio-TC apresenta resultados satisfatórios em comparação com os da SPECT na detecção de DAC obstrutiva. A angio-TC de coronárias tem capacidade de afastar falso-positivos da SPECT.

ABSTRACT

Abstract Coronary computed tomography angiography (CCTA) has gained a prominent role in the evaluation of coronary artery disease. However, its anatomical nature does not allow the evaluation of the functional repercussion of coronary obstructions. It has been made possible to evaluate Myocardial computed tomography perfusion (Myocardial CTP) recently, based on myocardial contrast changes related to coronary stenoses. Several studies have validated this technique against the anatomical reference method (cardiac catheterization) and other functional methods, including myocardial perfusion scintigraphy and fractional flow reserve. The Myocardial CTP is performed in conjunction with the CCTA, a combined analysis of anatomy and function. The stress phase (with assessment of myocardial perfusion) can be performed before or after the resting phase (assessment of resting perfusion and coronary arteries), and different acquisition parameters are proposed according to the protocol and type of equipment used. Stressors used are based on coronary vasodilation (e.g. dipyridamole, adenosine). Image interpretation, similar to other perfusion assessment methods, is based on the identification and quantification of myocardial perfusion defects. The integration of both perfusion and anatomical findings is fundamental for the examination interpretation algorithm, allowing to define if the stenoses identified are hemodynamically significant and may be related to myocardial ischemia.


Resumo A angiografia coronariana por tomografia computadorizada (ACTC) assumiu um papel de destaque na avaliação da doença arterial coronariana. Entretanto, sua natureza anatômica não permitia a avaliação da repercussão funcional das obstruções coronarianas. Recentemente, tornou-se possível a avaliação da perfusão miocárdica por tomografia computadorizada (PMTC), baseando-se nas alterações de contrastação miocárdicas relacionadas às estenoses coronarianas. Diversos estudos permitiram validar esta técnica perante o método anatômico de referência (cateterismo cardíaco) e outros métodos funcionais, incluindo cintilografia de perfusão miocárdica e a reserva de fluxo fracionada. A PMTC é realizada conjuntamente com a ACTC, em uma análise combinada de anatomia e função. A fase de estresse (com avaliação da perfusão miocárdica) pode ser realizada antes ou depois da fase de repouso (avaliação da perfusão de repouso e artérias coronárias), e diferentes parâmetros de aquisição são propostos conforme o protocolo e o tipo de equipamento utilizados. Os agentes estressores utilizados baseiam-se na vasodilatação coronariana (ex: dipiridamol, adenosina). A interpretação das imagens, semelhante a outros métodos de avaliação perfusional, baseia-se na identificação e quantificação de defeitos de perfusão miocárdicos. A integração dos achados perfusionais e anatômicos é parte fundamental do algoritmo de interpretação do exame, permitindo definir se as estenoses identificadas são hemodinamicamente significativas, podendo se relacionar com isquemia miocárdica.

Abstract Background: Obesity is associated with an increased risk of type 2 diabetes mellitus (DM), ischemic heart disease (IHD) and cardiovascular mortality. Several studies have demonstrated the diagnostic and prognostic value of single photon computed tomography-myocardial perfusion scintigraphy (SPECT-MPI) in the evaluation of patients with suspected IHD, including in obese population. Data on clinical risk factors and their association with abnormal myocardial perfusion in obese patients are scarce in the Brazilian population. Objective: To determine the factors associated with abnormal myocardial perfusion in obese individuals without known IHD. Methods: We studied obese patients without known IHD who were referred for evaluation through SPECT-MPI between January 2011 and December 2016. Clinical variables and results of SPECT-MPI were obtained systematically. The distribution of continuous variables was assessed using the Shapiro-Wilk and Shapiro-Francia tests. We used the unpaired Student t test to compare the means of continuous variables with normal distribution and the Chi Square test for binomial variables analysis. A p value < 0.05 was considered statistically significant. The association of the clinical variables for the presence of factors associated with abnormal myocardial perfusion was determined by univariate and multivariate logistic regression analysis, and respective odds ratios (OR) and 95% confidence intervals (CI). Results: The study sample consisted of 5,526 obese patients. Mean body mass index (BMI) of our patients was 33.9 ± 3.7 kg/m2, 31% had DM, and myocardial perfusion abnormalities was observed in 23% of the total sample. The factors associated with abnormal myocardial perfusion on multivariate analysis were: age (OR: 1.02, 95% CI 1.01-1.03, p < 0.001), DM (OR: 1.57, 95% CI 1.31-1.88, p < 0.001), typical angina before the test (OR: 2.45, 95% CI: 1.82-3.31, p < 0.001), need for pharmacologic stress test (OR: 1.61, 95% CI: 1.26-2.07, p < 0.001), less physical effort evaluated in metabolic equivalents (METs) during the exercise treadmill test (OR: 0.89, 95% CI: 0.85-0.94, p < 0.001) and a lower post-stress left ventricular ejection fraction after stress (LVEF; OR: 0.989, 95% CI: 0.984-0.994, p < 0.001). Conclusion: The factors associated with abnormal myocardial perfusion in obese patients without known IHD were age, DM, presence of typical angina, ventricular dysfunction, and inability to undergo physical stress as clinical variables, in addition to functional capacity during physical stress.


Resumo Fundamento: A obesidade associa-se a um risco aumentado de diabetes mellitus do tipo 2 (DM), doença cardíaca isquêmica (DCI) e mortalidade cardiovascular. Vários estudos demonstraram o valor diagnóstico e prognóstico da cintilografia de perfusão miocárdica com tomografia computadorizada por fóton único (CPM-SPECT) na avaliação de pacientes com suspeita de DCI, inclusive na população de obesos. Dados sobre fatores de risco clínicos, e sua associação com perfusão miocárdica anormal em obesos, são escassos na população brasileira. Objetivo: Determinar quais são os fatores associados à anormalidade de perfusão miocárdica em obesos sem DCI conhecida. Métodos: Estudamos pacientes obesos sem DCI conhecida que foram encaminhados para avaliação por CPM-SPECT entre janeiro de 2011 até dezembro de 2016. Variáveis clínicas e resultados da CPM-SPECT foram obtidos de forma sistematizada. A distribuição das variáveis contínuas foi avaliada utilizando-se os testes de Shapiro-Wilk e Shapiro-Francia. Utilizou-se o teste t de Student não pareado para comparar as médias das variáveis contínuas com distribuição normal, e o teste do Chi quadrado para análise das variáveis binomiais. Considerou-se o valor de p < 0,05 como estatisticamente significativo. A associação das variáveis clínicas para a presença de anormalidade de perfusão miocárdica foi determinada por análise de regressão logística univariada e multivariada, calculando-se e apresentando-se os respectivos odds ratios (OR) e intervalos de confiança (IC) de 95. Resultados: A amostra do estudo foi de 5.526 pacientes obesos. O índice de massa corporal médio dos nossos pacientes foi de 33,9 ± 3,7 kg/m2, 31% eram portadores de DM, e anormalidades de perfusão foram observadas em 23% do total da amostra. Os fatores associados à anormalidade de perfusão miocárdica, após análise multivariada, foram: idade (OR: 1,02, IC 95%: 1,01-1,03, p < 0,001), DM (OR: 1,57, IC 95%: 1,31-1,88, p < 0,001), presença de angina típica antes do exame (OR: 2,45, IC 95%: 1,82-3,31, p < 0,001), necessidade de utilização de protocolo com estresse farmacológico (OR: 1,61, IC 95%: 1,26-2,07, p < 0,001), menor esforço físico avaliado em equivalentes metabólicos durante o teste ergométrico (OR: 0,89, IC 95%: 0,85-0,94, p < 0,001) e menor fração de ejeção do ventrículo esquerdo após estresse (OR: 0,989, IC 95%: 0,984-0,994, p < 0,001). Conclusão: Os fatores associados à anormalidade de perfusão miocárdica em pacientes obesos sem DCI conhecida foram idade, DM, presença de angina típica, disfunção ventricular e incapacidade para o estresse físico como variáveis clínicas, além da capacidade funcional durante o estresse físico.

ABSTRACT

A miocardiopatia hipertrófica é a cardiopatia hereditária mais comum e acomete cerca de 1:500 indivíduos na população geral. O diagnóstico, que nem sempre é simples pela variação fenotípica e pela concomitância de outras patologias, baseia-se, inicialmente, em critérios eletrocardiográficos e ecocardiográficos, e na ausência de outras doenças que cursem com hipertrofia ventricular. Tendo como base celular o desarranjo miofibrilar e a fibrose com alterações hemodinâmicas decorrentes, a miocardiopatia hipertrófica pode revelar isquemia miocárdica (não relacionada à aterosclerose) e morte súbita. Assim, a avaliação da repercussão funcional por meio da cintilografia de perfusão miocárdica pela técnica Single Photon Emission Computed Tomography (SPECT) tem ganhado espaço, uma vez que 25% dos pacientes com miocardiopatia hipertrófica demonstram defeitos de perfusão fixos ou isquêmicos. Neste âmbito, notam-se alterações perfusionais que não estão necessariamente associadas ao tipo de miocardiopatia hipertrófica, mas conseguem predizer morbimortalidade nestes indivíduos. Outra técnica cintilográfica mais recente é a tomografia por emissão de pósitrons (PET), que se destaca na avaliação da microcirculação, na reserva de fluxo coronário e no metabolismo miocárdico. Em pacientes com miocardiopatia hipertrófica, estudos têm demonstrado resultados desfavoráveis quanto menores o fluxo sanguíneo miocárdico e a reserva de fluxo coronário. A avaliação miocárdica metabólica pela PET parece útil no entendimento fisiopatológico desta doença e na avaliação prognóstica da ablação alcoólica, procedimento realizado em formas obstrutivas graves. Assim, esta revisão aborda o papel da cardiologia nuclear pelas técnicas SPECT e PET miocárdico na avaliação diagnóstica, prognóstica e terapêutica da miocardiopatia hipertrófica

ABSTRACT

Abstract Background: Patients with ST-elevation acute myocardial infarction attending primary care centers, treated with pharmaco-invasive strategy, are submitted to coronary angiography within 2-24 hours of fibrinolytic treatment. In this context, the knowledge about biomarkers of reperfusion, such as 50% ST-segment resolution is crucial. Objective: To evaluate the performance of QT interval dispersion in addition to other classical criteria, as an early marker of reperfusion after thrombolytic therapy. Methods: Observational study including 104 patients treated with tenecteplase (TNK), referred for a tertiary hospital. Electrocardiographic analysis consisted of measurements of the QT interval and QT dispersion in the 12 leads or in the ST-segment elevation area prior to and 60 minutes after TNK administration. All patients underwent angiography, with determination of TIMI flow and Blush grade in the culprit artery. P-values < 0.05 were considered statistically significant. Results: We found an increase in regional dispersion of the QT interval, corrected for heart rate (regional QTcD) 60 minutes after thrombolysis (p = 0.06) in anterior wall infarction in patients with TIMI flow 3 and Blush grade 3 [T3B3(+)]. When regional QTcD was added to the electrocardiographic criteria for reperfusion (i.e., > 50% ST-segment resolution), the area under the curve increased to 0.87 [(0.78-0.96). 95% IC. p < 0.001] in patients with coronary flow of T3B3(+). In patients with ST-segment resolution >50% and regional QTcD > 13 ms, we found a 93% sensitivity and 71% specificity for reperfusion in T3B3(+), and 6% of patients with successful reperfusion were reclassified. Conclusion: Our data suggest that regional QTcD is a promising non-invasive instrument for detection of reperfusion in the culprit artery 60 minutes after thrombolysis.


Resumo Fundamento: Pacientes com infarto do miocárdico com elevação do segmento-ST atendidos em centros de atendimento primário e tratados de acordo com a estratégia fármaco-invasiva são submetidos à fibrinólise seguida de coronariografia em período de 2-24h. Neste cenário, o conhecimento de marcadores de reperfusão como a redução em 50% do segmento-ST é fundamental. Objetivo: Analisar o desempenho da dispersão do intervalo QT em adição aos critérios clássicos, como marcador precoce de reperfusão pós-terapia trombolítica. Métodos: Estudo observacional com a inclusão de 104 pacientes tratados com tenecteplase (TNKase) e referenciados a hospital de atendimento terciário. A análise dos eletrocardiogramas (ECG) consistiu em mensuração do intervalo QT e sua dispersão nas 12 derivações, e também apenas na região com supradesnivelamento-ST antes e 60min pós-TNKase. A angiografia foi realizada em todos os pacientes com obtenção do fluxo TIMI e Blush da artéria culpada. Foram considerados significantes valores de p < 0,05. Resultados: Observamos aumento da dispersão do intervalo QT, corrigido pela frequência cardíaca, regional (dQTcR) 60min pós-lise (p = 0,006) em infartos de parede anterior nos casos com fluxo TIMI 3 e Blush 3 [T3B3(+)]. Adicionando a dQTcR ao critério ECG (redução do ST > 50%) de reperfusão, a área sob a curva aumentou para 0,87 [(0,78-0,96), IC95%, p < 0,001] em pacientes com fluxo coronário T3B3(+). Nos pacientes com critério de ECG para reperfusão e dQTcR > 13 ms a sensibilidade e especificidade foram 93% e 71%, respectivamente, para reperfusão em T3B3(+), possibilitando reclassificar 6% dos pacientes com sucesso de reperfusão. Conclusão: Os dados sugerem a dQTcR como instrumento promissor na identificação não invasiva de reperfusão na artéria coronária culpada, 60min pós-trombólise.

Abstract Background: The role of myocardial perfusion scintigraphy (MPS) in the follow-up of asymptomatic patients after percutaneous coronary intervention (PCI) is not established. Objectives: To evaluate the prognostic value and clinical use of MPS in asymptomatic patients after PCI. Methods: Patients who underwent MPS consecutively between 2008 and 2012 after PCI were selected. The MPS were classified as normal and abnormal, the perfusion scores, summed stress score (SSS), and summed difference score (SDS) were calculated and converted into percentage of total perfusion defect and ischemic defect. The follow-up was undertaken through telephone interviews and consultation with the Mortality Information System. Primary endpoints were death, cardiovascular death, and nonfatal acute myocardial infarction (AMI), and secondary endpoint was revascularization. Logistic regression and COX method were used to identify the predictors of events, and the value of p < 0.05 was considered statistically significant. Results: A total of 647 patients were followed for 5.2 ± 1.6 years. 47% of MPS were normal, 30% were abnormal with ischemia, and 23% were abnormal without ischemia. There were 61 deaths, 27 being cardiovascular, 19 non-fatal AMI, and 139 revascularizations. The annual death rate was higher in those with abnormal perfusion without ischemia compared to the groups with ischemia and normal perfusion (3.3% × 2% × 1.2%, p = 0.021). The annual revascularization rate was 10.3% in the ischemia group, 3.7% in those with normal MPS, and 3% in those with abnormal MPS without ischemia. The independent predictors of mortality and revascularization were, respectively, total perfusion defect greater than 6%, and ischemic defect greater than 3%. Forty-two percent of the patients underwent MPS less than 2 years after PCI, and no significant differences were observed in relation to those who underwent it after that period. Conclusion: Although this information is not contemplated in guidelines, in this study MPS was able to predict events in asymptomatic after PCI patients, regardless of when they were performed.


Resumo Fundamentos: O papel da cintilografia de perfusão miocárdica (CPM) no seguimento de pacientes assintomáticos após intervenção coronariana percutânea (ICP) não está estabelecido. Objetivos. Avaliar o valor prognóstico e o uso clínico da CPM em pacientes assintomáticos após ICP. Métodos: Foram selecionados pacientes que realizaram CPM consecutivamente entre 2008 e 2012 após ICP. As CPM foram classificadas em normais e anormais, os escores de perfusão, escore somado do estresse (SSS) e escore somado da diferença (SDS) foram calculados e convertidos em porcentagem de defeito perfusional total e de defeito isquêmico. O seguimento foi por meio de entrevistas telefônicas e consulta ao Sistema de Informação de Mortalidade. Desfechos primários foram morte, morte cardiovascular e infarto agudo do miocárdio (IAM) não fatal e desfecho secundário foi revascularização. Regressão logística e método de COX foram utilizados para identificar os preditores de eventos e o valor de p < 0,05 foi considerado estatisticamente significativo. Resultados: 647 pacientes foram acompanhados por 5,2 ± 1,6 anos. 47% das CPM foram normais, 30% anormais com isquemia e 23% anormais sem isquemia. Ocorreram 61 mortes, 27 cardiovasculares, 19 IAM não fatais e 139 revascularizações. A taxa anual de óbitos foi superior naqueles com perfusão anormal sem isquemia comparada aos grupos com isquemia e perfusão normal (3,3% × 2% × 1,2%, p = 0,021). A taxa anual de revascularização foi 10,3% no grupo com isquemia, 3,7% naqueles com CPM normal e 3% naqueles com CPM anormal sem isquemia. Foram preditores independentes de mortalidade e revascularização, respectivamente, defeito perfusional total maior que 6% e defeito isquêmico maior que 3%. Quarenta e dois por cento dos pacientes realizaram CPM menos de 2 anos após ICP e não foram observadas diferenças relevantes em relação aos que realizaram após esse período. Conclusão: Embora esta informação não esteja contemplada em diretrizes, neste estudo a CPM foi capaz de predizer eventos em pacientes assintomáticos após ICP, independente do momento de realização.

Background: Dipyridamole (DIP) is the most commonly employed pharmacological stressor for myocardial perfusion tomography (SPECT) in patients unable to reach an adequate work load. Aim: To assess the predictive capacity of DIP SPECT on survival. Material and Methods: We included 985 adults aged 66 ±11 years (45% women) with rest and DIP-SPECT. The main indications for the procedure were coronary artery disease (CAD) screening in 66% and known CAD in 33%. Participants were followed up for a median of 65 months (interquartile range 54 to 86 months). During the follow up, 261 deaths were recorded and 98% had a specified cause in their death certificate. Results: Myocardial SPECT was abnormal in 44% of participants. Transient ischemic defects were observed in 34%, fixed defects concordant with infarction in 27% and post-stress systolic dysfunction in 23%. Twenty five percent of deaths were attributable to cardiac or ischemic cause and 22% to cancer. In a bivariate analysis, the hazard ratio (HR) of death of any cause was lower in females and higher in the presence of CAD. The multivariate analysis showed that being older than 46 years increased the HR of death of any cause. In a bivariate analysis, the HR for cardiac death was higher when the myocardial SPECT showed ischemia, necrosis or left ventricular dilation. In the multivariate analysis, post-stress left ventricular systolic function was associated with a lower risk of cardiac death. Conclusions: An abnormal myocardial SPECT, perfusion abnormalities, left ventricular systolic function or dilation are independent predictors of cardiac death in these participants.

Fundamentos: Cerca de 20 a 40% dos pacientes com insuficiência cardíaca não respondem à terapia de ressincronização. Para melhorar a seleção de pacientes, foi desenvolvida a análise de fase pela cintilografia de perfusão miocárdica (GSPECT). Objetivo: Avaliar a resposta clínica e cintilográfica de pacientes com insuficiência cardíaca submetidos à terapia de ressincronização pelo uso do GSPECT. Métodos: Estudo intervencionista, incluindo pacientes consecutivos que foram estudados pelo GSPECT 4 semanas antes da implantação da terapia de ressincronização cardíaca e 6 meses depois do implante. Estes pacientes responderam ao Questionário de Qualidade de Vida de Minnesota. A comparação das variáveis categóricas foi realizada com teste exato de Fisher e teste qui quadrado e, em variáveis numéricas, com o teste t de Student. O nível de significância estatística adotado foi de 5%. As variáveis cintilográficas analisadas foram fração de ejeção do ventrículo esquerdo, volume sistólico final, volume diastólico final, massa do ventrículo esquerdo, desvio padrão e largura de banda, assim como a duração do QRS e o escore do Questionário de Qualidade de Vida de Minnesota. A presença de dissincronismo mecânico foi definida como desvio padrão > 43º.Resultados: Foram incluídos nove pacientes no estudo. Após a terapia de ressincronização cardíaca, houve melhora significativa (p < 0,05) dos parâmetros volume sistólico final (206 ± 80 mL vs. 158 ± 108 mL), QRS (180 ± 18 ms vs. 120 ± 9 ms), massa do ventrículo esquerdo (248 ± 65 g vs. 193 ± 52 g) e Questionário de Qualidade de Vida de Minnesota (63 ± 16 vs. 34 ± 20). Todos os pacientes com critérios cintilográficos de dissincronismo mecânico apresentaram melhora clínica. Dois pacientes apresentavam apenas dissincronismo elétrico e não atingiram melhora clínica significativa, apesar de apresentarem redução da duração do QRS. Conclusão: O GSPECT foi capaz de diferenciar os pacientes com dissincronismo elétrico puro daqueles com dissincronismo mecânico associado, por meio dos parâmetros de dissincronismo intraventricular. A terapia de ressincronização cardíaca está associada à melhora tanto do dissincronismo mecânico quanto do elétrico. O GSPECT pré-implante identificou que os pacientes com dissincronismos elétrico e mecânico associados apresentaram melhor resposta à terapia de ressincronização cardíaca do que aqueles com dissincronismo elétrico puro


Background: Approximately 20 to 40% of patients with heart failure do not respond to cardiac resynchronization therapy (CRT). To improve patient selection, phase analysis by myocardial perfusion scintigraphy (GSPECT) was developed. Objectives: To evaluate the clinical and scintigraphic response of patients with heart failure (HF) submitted to CRT using GSPECT. Method: This was an interventional study that included consecutive patients assessed by GSPECT four weeks prior to CRT implantation and six months after it for comparison. These patients also answered the Minnesota Living with Heart Failure Questionnaire (MLHFQ). The categorical variables were compared using Fisher's exact test and chi-square test, whereas Student's t-test was used for numerical variables. The level of statistical significance was set at 5%. The scintigraphic variables analyzed were left ventricular ejection fraction, end-systolic volume, end-diastolic volume, left ventricular mass, standard deviation and bandwidth, as well as QRS duration and the Minnesota Quality of Life Questionnaire score. The presence of mechanical dyssynchrony was defined as standard deviation > 43º. Results: Nine patients were included in the study. After the cardiac resynchronization therapy, there was a significant improvement (p <0.05) in the end-systolic volume (206 ± 80 mL vs. 158 ± 108 mL), QRS (180 ± 18 ms vs. 120 ± 9 ms), left ventricular mass (248 ± 65 g vs. 193 ± 52 g) and Minnesota Quality of Life Questionnaire score (63 ± 16 vs. 34 ± 20). All patients with scintigraphic criteria of mechanical dyssynchrony showed clinical improvement. Two patients had only electrical dyssynchrony and did not achieve significant clinical improvement, although they showed QRS duration reduction. Conclusion: GSPECT was able to differentiate patients with isolated electrical dyssynchrony from those with associated mechanical dyssynchrony, through the intraventricular dyssynchrony parameters. The cardiac resynchronization therapy is associated with the improvement of both mechanical and electrical dyssynchrony. Pre-implantation GSPECT showed that patients with associated electrical and mechanical dyssynchrony had a better response to cardiac resynchronization therapy than those with isolated electrical dyssynchrony

Lesões coronárias moderadas podem ser, ou não, responsáveis pela isquemia miocárdica. A análise funcional das lesões pode ser realizada por métodos invasivos e não invasivos. Comparar a análise funcional das lesões coronarianas moderadas pela reserva de fluxo fracionado e pela cintilografia de perfusão miocárdica. Foram estudados prospectivamente 47 pacientes com doença arterial coronária estável com pelo menos uma lesão coronariana moderada obstrutiva. Eles foram submetidos à reserva de fluxo fracionado e à cintilografia de perfusão miocárdica com intervalo médio de 24,5 dias, entre janeiro de 2013 e dezembro de 2015. Não houve alteração no estado clínico e nem no procedimento de revascularização entre exames. As variáveis populacionais foram descritas como mediana e interquartil. A reserva de fluxo fracionado foi realizada em um de tronco de coronária esquerda; 37 artérias coronárias descendentes; 12 artérias circunflexas e quatro artérias coronárias direitas. Reserva de fluxo fracionado < 0,8 foi considerada positiva. A análise comparativa entre os resultados dos testes foi feita pelo teste de Fisher bicaudal, sendo considerado significativo valor de p < 0,05. A reserva de fluxo fracionado < 0,8 foi encontrada no tronco de coronária esquerda (100%); 13 na artéria coronária descendente (35,14%); seis na artéria circunflexa (50%) e duas na artéria coronária direita (50%). Dentre os pacientes com reserva de fluxo fracionado positiva, 83% tinham isquemia miocárdica demonstrada na cintilografia de perfusão miocárdica (p = 0,058). Analisando especificamente o território da artéria coronária descendente, 83% dos pacientes com reserva de fluxo fracionado negativa não tinham isquemia na cintilografia de perfusão miocárdica, mas 69% dos pacientes com reserva de fluxo fracionado positiva não tinham isquemia na cintilografia de perfusão miocárdica (p = 0,413). Pode ocorrer discordância entre os resultados de análise funcional de lesões coronárias moderadas por testes invasivos e não invasivos


Moderate coronary artery lesions can be, or not, responsible for myocardial ischemia. The functional analysis of these lesions can be performed by invasive and noninvasive methods.To compare the functional analysis of moderate coronary lesions by fractional flow reserve and myocardial perfusion scintigraphy. 47 patients with stable coronary artery disease and at least one moderate coronary artery obstruction were prospectively studied. They were submitted to fractional flow reserve and myocardial perfusion scintigraphy with a median interval of 24.5 days between January 2013 and December 2015. There was no change in clinical status or revascularization procedure between the exams. The population variables were described as medians and interquartile range. Fractional flow reserve was performed in one left main coronary artery; 37 left descending coronary arteries; 12 circumflex arteries and 4 right coronary arteries. Fractional flow reserve < 0.8 was considered positive. The comparative analysis between the results of the tests was performed by two-tailed Fisher's test and a p-value 0.05 was considered significant.Fractional flow reserve < 0.8 was found in the left main coronary artery (100%); 13 in the left descending coronary artery (35.14%); 6 in circumflex artery (50%) and 2 in the right coronary artery (50%). Among the patients with positive fractional flow reserve, 83% had myocardial ischemia demonstrated by the myocardial perfusion scintigraphy (p = 0.058).When analyzing specifically the left descending coronary artery, 83% of the patients with negative fractional flow reserve showed no ischemia at the myocardial perfusion scintigraphy, but 69% of the patients with positive fractional flow reserve showed no ischemia at the myocardial perfusion scintigraphy (p = 0.413). Disagreements can occur between the results of the functional analysis of moderate coronary lesions by invasive and noninvasive tests

Abstract Background: Data on the current situation of nuclear medicine practices in cardiology in Brazil are scarce. The International Atomic Energy Agency (IAEA) has recommended eight "good practices" to minimize patients' ionizing radiation exposure during myocardial perfusion scintigraphy (MPS). Objectives: To assess the adoption of the eight good practices in MPS in Brazil. Methods: Cross-sectional study with data obtained by use of a questionnaire. All hypothesis tests performed considered a significance level of 5%. Results: We observed that 100% of the nuclear medicine services (NMS) assessed do not use thallium-201 as the preferred protocol. Regarding the use of technetium-99m, 57% of the NMS administer activities above the threshold recommended by the IAEA (36 mCi) or achieve an effective dose greater than 15 millisievert (mSv). The abbreviated stress-only myocardial perfusion imaging is not employed by 94% of the NMS; thus, only 19% count on strategies to reduce the radioactive doses. Approximately 52% of the NMS reported always performing dose adjustment for patient's weight, while 35% administer poorly calculated doses in the one-day protocol. Conclusion: A considerable number of NMS in Brazil have not adopted at least six practices recommended by the IAEA. Despite the difficulties found in nuclear practice in some Brazilian regions, almost all obstacles observed can be overcome with no cost increase, emphasizing the importance of developing strategies for adopting "good practices" when performing MPS.


Resumo Fundamento: A situação atual das práticas da medicina nuclear em cardiologia no Brasil ainda é pouco conhecida. A Agência Internacional de Energia Atômica (AIEA) recomendou oito "boas práticas" para minimizar a exposição dos pacientes à radiação ionizante durante a Cintilografia de Perfusão Miocárdica (CPM). Objetivo: Analisar a adoção das oito boas práticas na CPM no Brasil. Métodos: Estudo transversal com dados obtidos através de questionário. Todos os testes de hipóteses desenvolvidos consideraram uma significância de 5%. Resultados: Observamos que 100% dos Serviços de Medicina Nuclear (SMN) não utilizam Tálio-201 como protocolo preferencial. Sobre a utilização do Tecnécio-99m, notamos que 57% administram atividades acima do limiar recomendado pela AIEA (36 mCi) ou resultam em uma dose efetiva maior que 15 milisievert (mSv). A fase única de estresse não é praticada por 94% dos SMN; portanto, somente 19% contam com estratégias de redução das doses radioativas. Cerca de 52% dos SMN afirmam que sempre realizam o ajuste da dose por peso e 35% administram doses mal calculadas no protocolo de um dia. Conclusão: Observamos que um número considerável de SMN no Brasil ainda não seguem seis ou mais das práticas recomendadas pela AIEA. Apesar das dificuldades enfrentadas na prática nuclear em algumas regiões do Brasil, quase todos os déficits observados podem ser resolvidos sem aumento de custos, ressaltando a importância do desenvolvimento de estratégias para aderência às "boas práticas" na realização da CPM.

ABSTRACT

Resumen Objetivo: Durante el período de formación, el residente de cardiología ha de entrenarse en todo lo relacionado con la indicación, la interpretación y realización de imágenes en cardiología nuclear que usan la tomografía computarizada por emisión de fotón simple (SPECT). El objetivo del presente estudio fue analizar la relación existente entre la adecuación de las indicaciones del gated-SPECT de perfusión miocárdica y los años de experiencia desde la finalización de la residencia de cardiología. Método: Registro descriptivo, retrospectivo y unicéntrico, en el que se analizaron las indicaciones (uso adecuado e inadecuado), según las guías de la gated-SPECT de perfusión miocárdica, prescritas por cardiólogos de un hospital universitario. Resultados: Se analizaron un total de 950 pruebas de gated-SPECT de acuerdo a la indicación adecuada e inadecuada y se distribuyeron por cuartiles los años de finalización de la residencia del cardiólogo prescriptor. Los cardiólogos de menos de 10 años de experiencia clínica indican de forma inadecuada una mayor proporción de pruebas de gated-SPECT que los cardiólogos de mayor experiencia (87,6 vs. 9,3%: p < 0,001). Tras ajustar por edad, sexo y factores de riesgo cardiovascular, el análisis multivariante mostró que por cada año de experiencia tras finalizar la residencia de cardiología aumenta un 33% la probabilidad de indicar de forma adecuada la prueba (OR: 1,33; IC 95%: 1,29-1,38; p < 0,001). Conclusiones: La experiencia profesional del cardiólogo clínico es el factor más importante para realizar una indicación adecuada de la gated-SPECT de perfusión miocárdica.


Abstract Objective: During cardiology training, the cardiology fellow has to be trained in all things related to the indication, interpretation, and performing of nuclear cardiology studies using single photon emission computed tomography (SPECT). The aim of the present study was to analyse the relationship between the adequacy of indications of myocardial perfusion gated-SPECT and the years of experience since the completion of cardiology training. Method: A descriptive, retrospective analysis was performed on a single-centre register, in which the indications (adequate or inadequate use) were recorded according to myocardial perfusion gated-SPECT guidelines, prescribed by cardiologists of a university hospital. Results: A total of 950 gated-SPECT tests were analysed according to the appropriate or inade- quate indication. The sample of study was distributed in quartiles (years) since the cardiologist finished the residency. Cardiologists with less than 10 years of clinical experience reported a higher proportion of gated-SPECT tests compared to the more experienced cardiologists (87.6 vs. 9.3%, P < .001). After adjusting for age, gender, and cardiovascular risk factors, the multiva- riate analysis showed that, for each year of experience after completion of cardiology training, the probability of adequately indicating the test (OR: 1.33, 95% CI: 1.29-1.38, P < .001) was statistically significant. Conclusions: The professional experience of the clinical cardiologist is the most important factor to perform an appropriate indication of gated-SPECT myocardial perfusion.
